Open communication, personal attention and shared decision-making provide the foundation for your surgical plan. We take the time to thoroughly explain all procedures and surgeries with you and your family.
We participate in some insurance plans. If you are not insured by a plan we are contracted with, payment in full is expected at time of visit. We are NOT contracted with Medi-Cal. If you are insured by a plan we are contracted with but don’t have an up-to-date insurance card, payment in full for each visit is required until we can verify your coverage.
Some insurance carriers require you to bring a referral or authorization for your visit before payment for your treatment can be given. It is your responsibility to insure that we have this authorization before your visit. We cannot obtain a referral at the time of your visit. Please check with your carrier.
Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage.
Please call the office, (562) 314-1400, Ext. 2, to make an appointment in advance. You may also request an appointment online.
Kindly have your insurance card available when you make the appointment. Appointments cannot be confirmed until insurance coverage is verified. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
In order to provide our patients with timely scheduling options, the following office policies are now in place. Please become familiar with them:
We request payment for all office services at the time they are rendered. All co-pays and coinsurance payments are due at the time of your visit. Patients will not be seen unless their accounts are considered current. For your convenience we accept cash, checks, MasterCard, Visa, and Discover.
As a courtesy to you, if we are contracted with your insurance company, we will bill them. However, payment is the patient’s responsibility. Any amount not received from the insurance company becomes the patient responsibility and is due and payable immediately. If we are not contracted with your insurance company, payment in full for services rendered is collected at the completion of your office visit. We will supply you with a conforming copy of the medical superbill that may be submitted to the insurance company for reimbursement by the patient. We will help in any way we can to assist you in handling claims.
If you have a prescription that needs to be refilled, please call the pharmacy that last filled your medication at least 4 days before you will need the medication. All prescriptions are processed electronically through your pharmacy, we do not process phone requests. If you have not seen the doctor for more than 90 days, you will need to schedule an appointment to see the doctor before a prescription is refilled. This is a state mandated regulation. If you are in immediate need of a prescription there will be a $10 fee to hand write a prescription for pick up.
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
